RN Care Coordination & Care Coordination Services

SJ Mobile Nursing provides RN care companion services designed to support patients and families during medical appointments and care transitions.

This service is ideal for seniors, homebound individuals, and patients who feel overwhelmed during healthcare visits or whose family members cannot be present. We also offer nursing home visits!

Services may include:

• Appointment accompaniment

• Documentation of key visit details

• Clarification of provider instructions

• Communication with family members or caregivers

• Family requested independent nursing check in visits for reassurance when loved ones cannot be present (including nursing home or assisted living settings)

RN care companion services focus on support, organization, and communication. This service does not include medical diagnosis or prescribing.
